I can imagine at the end of realization when the main character shouts 'let me out!' he'd sit up and the lights would go out on them at the same time. To which he just angry collapses back into bed.

Getting actions right would be difficult obviously, but rewarding. I suspect some solos would have to be either cut down or cut out simply to help the actors work. Still, in Love for example, I can imagine Reason dancing with Passion and so Pride dances with Love purely to show himself better. And as the song ends all the lights go out but for the spotlight on the main character and the wife.

I was thinking pretty much along those lines. It would be incredibly hard to put together, but so very very worthwhile.

The way I see it the band would most definately be at the back. To the left of the stage you'd have your main character in bed with the wife and best friend on either side. The lighting (on the main character with the others blacked out/all lit up with the beeping machine and all) would tell you which world you were in at the time. I imagine between Vigil and Isolation the main character would literally get out of bed and sort of have an 'out of body experience.' This also allows him to communicate with the other characters.

Female characters are of course more difficult what with there being only three of them. I like Passion's first piece but after that she gets rather stale. Love is a rarity but I think I prefer her because when she talks she talks absolute sense.

I've always been desperate to see it live. This is, of course, completely impossible, but it would be awesome. Perhaps when I get a band together and do concept album gigs, (Verity does Dream Theater's Scenes From a Memory for example) I'd be able to get together singers to perform it.
